Home Care Packages (HCP) Daylesford and Central Highlands

 

 

Nightingale Home Care is now providing In-Home care to clients with HCP funding in the Daylesford and Central Highlands region.

Home Care Packages (HCP) support the ageing population with complex needs to stay at home as long as possible.

If you or a loved one has HCP funding, you are able to work with service providers to plan, organise and deliver services you need the most.

Nightingale Home Care currently assists HCP clients through brokerage agreements with larger providers.

What does this mean?
Nightingale Home Care is not yet an approved provider of Aged care Services. However, if you are contracted with another provider in a “self-managed” capacity, you are allowed to choose your own care and nursing staff – this is where we come in.

Nightingale Home Care is able to provide all in-home care services including but not limited to: personal hygiene and dressing, community access, meal preparation, transport to appointments, nursing services including wound management and catheter care.
